Open roles
Addiction recovery professional who is part of an interdisciplinary team providing recovery-oriented services in an OASAS 822 clinic for individuals with substance use and co-occurring disorders.
Minimum required credentials: LMSW, LMSW-P, LMHC, LMHC-P, LCSW, or LMHC-D. Knowledge of OASAS regulations is required.
Management and administration of medication (oral & injectable) in conjunction with the psychiatrists; assessing physical health needs; making and assisting referral to appropriate community based health care providers. Demonstrate adequate knowledge of current DSM an understanding of AXIS I diagnosis, providing psycho-ed services in group, individual and family settings (ex: symptom management, health and wellness; medication education, smoking cessation). Work environment requires flexibility, with on and off site work locations, working as part of an interdisciplinary team and also individually.
Provide Personalized Recovery Oriented Services to program participants; includes individual and group counseling as well as case management/coordination of services. Conducts assessments, reviews, develops Individualized Recovery Plans and maintains documentation of clients’ participation and progress.
Reporting to the Director of Nursing, Nurse Supervisor, the Registered Nurse (RN) is responsible for the wellbeing of all of the people served under his/her care. Accordingly, the RN provides care management, medical consultation/triage services, medical education to the people served at the specified program and/or their family/guardians as well as education/training to and observation of staff members who have been delegated nursing duties. Medical care coordination is for all the people in the assigned program(s) is accomplished through ensuring that: appropriate medical treatment and follow-up are provided, training and observation of AMAP staff and the Certified Nurse Assistants in their performance of nursing duties are conducted, on-call consultation and triage are performed and participation in program planning and/or treatment plan meetings takes place.
