Home & Community-Based Services Waiver

THE HOME & COMMUNITY-BASED SERVICES WAIVER PROGRAM (HCBS)

The program is designed to help Seriously Emotionally Disturbed (SED) Suffolk County residents, 5 to 18 years of age, to remain at home by offering an array of community based services to prevent intermediate psychiatric admission or placement in a residential treatment facility. Services provided would enable a child already in such a placement to be discharged more quickly. The Waiver Program Services include: individualized care coordination, intensive in-home services, crisis response, skills building, respite care and family support/advocacy services.

Waiver services are culturally sensitive, individualized, strength-based focused to build resiliency, assist in the achievement of age related developmental tasks and promote emotional well-being. Services and supports are specifically tailored to the unique needs of the family

Waiver uses an integrated partnership model which includes an ongoing dialogue and participation by the family, treatment provider, core waiver services and other natural supports that are involved with the child and family.
